New patients are accepted as space is available.
To schedule an appointment please call the office during regular office hours. Routine visits will be set up on a timely bases. Emergency - same day appointments are available under most circumstances. An attempt will always be made to schedule you with your own health care provider but circumstances may sometimes dictate that your visit would be with one of our other practitioners.
Please bring your insurance cards with you to each of your appointments. Photocopies of your cards will be obtained for your file.
We participate with: Blue Cross Blue Shield Traditional Plan, Cigna Open Access Plus, Direct Care America, First Health, Great West Health Care, Medicare, MultiPlan/Real Health, PPOM, Priority Health HMO and Self-Funded Plans, Priority Health Medicare, Private Health Care Systems (PHCS), Secure One and United Health Care.
Office visit charges, co-pays and deductibles are payable at the time of your visit. While our business office personnel are available to assist you with insurance matters, the responsibility for payment remains solely with you - the patient. We will provide you with itemized receipts to submit to your insurance carrier for reimbursement.
For the protection of our patients, all patient records are held in strict confidence and will not be forwarded or released from our office without your written and signed consent.
